Juniper Gardens Center for Nursing & Rehabilitation is seeking a dedicated and compassionate Part-Time Physical Therapist (PT) to join our rehabilitation team. This is an excellent opportunity for a licensed Physical Therapist who is passionate about helping patients improve mobility, regain independence, and achieve meaningful functional goals.
As a Physical Therapist at Juniper Gardens, you will work collaboratively with patients, families, nursing staff, physicians, and the interdisciplinary rehabilitation team to provide individualized, goal-oriented care.
Key Responsibilities

Evaluate patients based on physician referrals, orders, and identified clinical needs

Develop individualized treatment plans with functional, measurable, and goal-oriented outcomes

Provide treatment in accordance with the established plan of care and professional standards

Monitor patient progress and modify treatment plans as clinically appropriate

Maintain accurate and timely clinical documentation in accordance with regulatory and professional requirements

Complete daily treatment documentation and charges according to applicable billing guidelines

Educate patients, families, and nursing staff on therapeutic techniques, restorative programs, and home exercise programs

Participate in discharge planning and help support successful transitions of care

Communicate effectively with the interdisciplinary team regarding patient progress, concerns, and treatment plans

Participate in interdisciplinary meetings, patient care conferences, utilization reviews, and other meetings as required

Participate in facility performance improvement initiatives and quality assurance programs
-
Provide education and in-service training to facility staff as appropriate

Supervise Physical Therapist Assistants, therapy students, rehab technicians, and other assigned personnel in accordance with applicable regulations

Screen patients and identify individuals who may benefit from physical therapy services

Coordinate with nursing, physicians, rehabilitation staff, and other facility departments

Maintain a clean, safe, and organized therapy environment

Assist with obtaining appropriate durable medical equipment and adaptive equipment for patients
-
Report equipment concerns and ensure therapy equipment remains safe and functional
-
Travel or float between nearby facilities as needed based on patient and operational needs
Qualifications
-
Graduate of an accredited Physical Therapy program
-
Current Physical Therapist license or eligibility for licensure in the state where services are provided

Strong clinical assessment and treatment skills

Excellent communication and interpersonal skills

Ability to work independently and as part of an interdisciplinary team

Strong organizational and documentation skills

Knowledge of Medicare and other applicable reimbursement requirements

Commitment to patient-centered care and positive clinical outcomes

Ability to work effectively with patients, families, and team members
Work Environment
The Physical Therapist will work primarily in therapy and patient care areas within a skilled nursing and rehabilitation setting. The position involves frequent interaction with patients, families, nursing staff, physicians, and other healthcare professionals.
The role may involve:

Sitting, standing, walking, bending, stooping, pushing, pulling, and lifting throughout the workday

Assisting with and safely supporting patients during treatment

Exposure to infectious diseases, body fluids, and other healthcare-related hazards

Working with patients who may be elderly, disabled, medically complex, emotionally upset, or occasionally difficult to manage
-
Occasional travel between facilities based on patient and operational needs
Physical Requirements

Ability to stand, walk, bend, stoop, push, pull, and sit for extended periods

Ability to safely assist, transfer, and support patients

Ability to lift and carry up to 50 pounds

Ability to communicate effectively with patients, families, and healthcare professionals

Ability to maintain appropriate attention to patient safety throughout treatment

Ability to manage the physical and emotional demands of the position
