Key Responsibilities

  • Coordinate and deliver high-quality, patient-focused care tailored to departmental needs and aligned with state and specialty practice standards.
  • Collaborate with medical professionals and care team members to provide personalized, comprehensive, and compassionate care using established nursing models such as “Assess, Perform, Teach, and Manage.”
  • Act as an advocate for patients, families, and caregivers, embodying the organization’s vision, mission, and values to enhance patient experiences and clinical outcomes.
  • Perform procedures, monitoring, and other tasks as directed by medical providers, ensuring timely and thorough documentation of care in patient medical records.
  • Educate patients, families, and caregivers about medical conditions, treatment plans, medications, potential side effects, and follow-up care, simplifying complex medical terminology for better understanding.
  • Implement evidence-based practices and continuous improvement initiatives to surpass quality measures and elevate the standard of care provided to patients.
  • Collaborate with a multidisciplinary team to design, evaluate, and deliver a holistic treatment plan that fosters recovery and well-being.

Qualifications

  • Registered Nurse Diploma required.
  • No travel is necessary for this position.
