What You’ll Need to Apply:

• Valid HHA or PCA Certification
• Valid ID (Driver’s License, Passport, or State ID)
• Social Security Card
• Employment Authorization Card (if required by your Social Security card)
• Pre-Employment Physical Form – must include MMR, TB test, and Drug Screen
  (We can provide our physical form if needed)
• COVID-19 Vaccine Card (if you received it)
• Flu Vaccine (if you received it)
• Hep B Vaccine (if you received it)
• Two (2) Work Related References
