Active LMSW (Licensed Master Social Worker) credential\n
Minimum 1 year of hospital or acute care experience required (acute rehab experience preferred)\n
ARU experience preferred but not required\n
Familiarity with EMR systems; Prod-Doc preferred\n
Ability to work full-time, on-site for the duration of the assignment\n

\n\nResponsibilities\n

Provide psychosocial support, patient advocacy, and comprehensive discharge planning for ARU patients\n
Perform clinical assessments, develop individualized care plans, and facilitate family meetings\n
Coordinate care and case management with interdisciplinary teams (nursing, therapy, physicians, and community resources)\n
Document assessments, interventions, and discharge plans accurately in the EMR system (Prod-Doc)\n
Serve as a resource for patients and families navigating rehabilitation, community referrals, and post-discharge services\n
