Schedule:

Flexible part-time (2–4 days/week, ~10–20 patient visits per week)

Job Summary:

We are seeking an experienced and compassionate Nurse Practitioner (NP) to deliver advanced wound care services in patients’ homes. This position involves evaluating, managing, and treating patients with acute and chronic wounds as part of a mobile wound care team. Ideal for a clinician seeking flexible hours with strong autonomy in a growing field.

Key Responsibilities:

• Conduct in-home patient assessments for chronic and complex wounds (e.g. pressure ulcers, diabetic ulcers, surgical wounds).
• Perform debridement, wound dressing changes, compression therapy, and negative pressure wound therapy.
• Develop individualized care plans and coordinate with primary care, podiatry, or vascular providers.
• Document care in Wound Expert, Tissue Analytics, and ECW.
• Educate patients and caregivers on wound care techniques and prevention.
• Collaborate with home health nurses and case managers.
• Maintain compliance with CMS, HIPAA, and state scope of practice regulations.

Qualifications:

• Active NP license in NY
• Board-certified Adult/Gerontology, Family NP, or Acute Care NP
• Wound care experience preferred (WCC, CWCN, or similar certifications a plus)
• Minimum 1–2 years of NP experience
• Reliable transportation and willingness to travel locally
• Strong documentation and clinical decision-making skills

Compensation:

• % based salary plus procedure bonuses
