Sugar Creek Care Center, a skilled nursing and rehabilitation facility in Pennsylvania, is seeking a dedicated and compassionate Physical Therapist (PT) to join our therapy team. This is a great opportunity for a clinician who is passionate about supporting short-term rehab and long-term care residents in achieving improved strength, mobility, and overall quality of life.
Key Responsibilities

Evaluate residents and develop individualized treatment plans.

Provide therapeutic interventions to improve mobility, strength, balance, and functional independence.

Document evaluations, progress notes, and treatment sessions accurately and timely.

Collaborate with an interdisciplinary team including nursing, OT, SLP, and social services.

Educate residents, families, and staff on home programs and functional goals.

Ensure compliance with all regulatory and clinical standards.
Qualifications

Active Physical Therapist license in Pennsylvania (required).

Experience in skilled nursing/long-term care preferred, but strong new grads encouraged to apply.

Excellent communication, assessment, and clinical reasoning skills.
