Job DescriptionSalary: $60.00/Hour

BASIC FUNCTION:



Provide pharmaceutical services for the Seneca Nation Health System (SNHS). Reports directly to Pharmacy Supervisor.



GENERAL RESPONSIBILITIES:

  1. Accurate filing and dispensing of prescription and over-the-counter medications on orders from both in-house providers as well as referred providers, including but not limited to medical, dental, optical, and behavioral health providers.
  2. Analyzes medication regimens to ensure most effective, safe, and economical treatment plan.
  3. Evaluates medication regimens for appropriateness (patient diagnoses, lab monitoring, contraindications, dose, schedule, length of therapy), allergies, drug interactions, therapeutic duplications, non-compliance, and adverse reactions.
  4. Counseling patients on the use, storage, and side effects of medications dispensed as well as documentation of educational services provided.
  5. Serves as a therapeutic consultant to medical staff and performs duties such as drug information, drug registry searches, medication reconciliation, etc.
  6. Maintains necessary records for controlled substances and other prescription and other prescription products as required by law.
  7. Communicates respectively and effectively with patients, community, and other SNHS employees.
  8. Participates in ongoing quality assurance and performance improvement activities including audits, chart reviews, medication interventions and error reporting, patient safety, formulary management and Pharmacy and Therapeutic activities.
  9. Provides formal and informal education and in-services to medical, pharmacy, and nursing staff in addition to patient groups as requested.
  10. Transmits pharmacy point-of-sale (POS) claims, assures appropriateness of billing, runs various POS reports, and reworks rejected claims with re-submittals.
  11. Attend all mandatory staff meetings and in-service trainings.
  12. Follow all policies and procedures of the department, SNHS, and Seneca Nation.

KNOWLEDGE, SKILLS, & ABILITIES:



  • Must have knowledge of the following: Planning, monitoring, and evaluating drug therapy in a clinical setting.
  • Use of the appropriate pharmacy resources and reference materials, including but not limited to Lexicomp, Facts & Comparisons, and Pharmacist Letter.
  • Ability to communicate with both patients and health care providers.
  • Ability to work independently, to plan, coordinate and implement projects and to complete projects on schedule.
  • Skills in problem solving.
  • Basic knowledge of management functions.



QUALIFICATIONS:



  • Bachelor of Science in Pharmacy with PGY-1 residency required, PGY-2 Preferred or Doctorate Degree in Pharmacy (PharmD) is required.
  • License to practice Pharmacy in New York State required.
  • 5 years of community pharmacy experience preferred, although, less experienced pharmacists who are committed to provisions of high quality pharmaceutical care will be considered.
  • Experience in a large hospital, academic medical center, outpatient health care setting, or Indian Health Service (IHS)/tribal health, preferred.
  • All required licenses, certifications, and credentials must be obtained prior to hire and maintained throughout employment at the employees expense, unless otherwise specified.
