Occupational Therapist (OT) – PACE Program
Location: Sacramento, CA

MedAdventures is seeking a skilled and compassionate Occupational Therapist (OT) to join a well-established PACE (Program of All-Inclusive Care for the Elderly) team in Sacramento, CA. This is an excellent opportunity for an OT who is passionate about helping older adults maintain independence, safety, and quality of life through community-based care.
In this role, you’ll work collaboratively with an interdisciplinary care team to evaluate, treat, and support participants in both clinic and home settings.

Key Responsibilities:
- Evaluate participants and develop individualized treatment plans to address ADLs, home safety, and mobility
- Provide skilled OT services in clinic and home environments
- Collaborate with PTs, SLPs, nursing, and primary care providers to support care goals
- Educate participants and caregivers on adaptive strategies, safety, and equipment use
- Document all care accurately and in compliance with regulatory guidelines
Qualifications:
- Active California OT license (or eligibility to obtain)
- Experience in geriatrics, home health, or PACE strongly preferred
- Excellent clinical judgment, communication, and documentation skills
- Commitment to person-centered care and aging-in-place support
