Position Summary

Under the supervision of the Care Coordination Outreach Supervisor, the Care Coordination Outreach Worker supports the delivery of coordinated healthcare services. This role is responsible for completing health risk assessments, conducting outreach, scheduling follow-up appointments after emergency department visits or inpatient discharges, and maintaining strong patient engagement while ensuring data accuracy in a fast-paced, hybrid environment.

Essential Duties & Responsibilities

  • Maintain patient confidentiality and adhere to all agency policies and ethical standards
  • Provide high-quality customer service using the AIDET model
  • Utilize Athena (EHR system) for documentation and outcomes tracking
  • Complete health risk screenings for assigned patients
  • Conduct outreach via phone and virtual platforms
  • Coordinate follow-up care after emergency department visits or inpatient discharges
  • Provide patient education on preventive care and appropriate healthcare utilization
  • Answer incoming calls, route appropriately, and document messages accurately
  • Collaborate with health plans, hospitals, physicians, and specialists
  • Attend required trainings, meetings, and core curriculum sessions
  • Perform additional duties as assigned

Qualifications

Education & Experience:

  • High School Diploma or equivalent required
  • Experience with Microsoft Office and data entry required

Additional Requirements:

  • Valid driver’s license and current auto insurance
  • Flexibility to work evenings, weekends, and holidays as needed
