Job DescriptionPrimary Care RN or LPN - Overview

The Primary Care Nurse at our organization collaborates with healthcare providers and clinical teams to ensure patient-focused, timely, and effective care. This role includes managing triage calls and enhancing quality outcomes for patients, with an option for hybrid work following the completion of initial on-site training. Under the guidance of the primary care operations team, the Primary Care Nurse directly engages with our Primary Care Providers to deliver safe, timely, effective, efficient, equitable, and patient-centered care systematically. This position involves partnering with patients, providers, and other clinical team members to achieve quality outcomes for an assigned patient panel. Responsibilities will be carried out in alignment with our mission, values, and service standards.

Schedule: Monday - Friday

No weekends or holidays!

**Completion of three months of on-site training is required before transitioning to a hybrid work-from-home schedule; availability for monthly clinic meetings and 1-2 on-site visits per week is also necessary.**

This newly established role for the clinics involves close collaboration with Providers and managing triage calls/messages. After training, there is an option to work in a hybrid capacity.


LPN Qualifications:

Education: Diploma in Practical Nursing

Licensure: Valid LPN license

Experience: Preferred 2-3 years of experience

RN Qualifications:

Education: Associate of Applied Science in Registered Nursing, graduation from a registered nursing program, a bachelor’s degree in nursing (BSN), or 4 years of relevant experience in lieu of a bachelor’s degree

Licensure: Valid RN license

Experience: Preferred 2-3 years of experience
