Care Management Coordinator
Onsite | Contract: 13 weeks | Start: ASAP

Weekend Requirement: Every Other Weekend
Job Summary
We are seeking a detail-oriented Care Management Coordinator to support the Case Management Department, consisting of 60 Case Managers and 5 Care Coordinators. The coordinator will provide essential clerical and administrative support under the supervision of licensed clinical staff, ensuring smooth communication between patients, healthcare providers, insurance companies, and other stakeholders. This role ensures safe and efficient transitions of care, proper authorization and payment for clinical services, and compliance with regulatory requirements.
Key Responsibilities

Assist with referrals to Skilled Nursing Facilities (SNF)

Initiate insurance authorizations

Arrange patient transportation

Deliver Medicare regulatory documents to patients

Support Case Management team with clerical and administrative tasks
Requirements
-
High School Diploma or GED required; Associate’s Degree in Business or Healthcare preferred

Epic experience required
-
Basic knowledge of medical terminology required

Experience with insurance authorizations preferred
