Job Title: Primary Care Nurse Practitioner – AGNP / CNP | Wound Care Training Covered

Compensation: $105,000 to $120,000 annually
Benefits: Full Benefits Package (for full-time status)
Location: Greater Twin Cities Metro Area, MN
Schedule: Full-Time

Position Summary:
This position is ideal for a Nurse Practitioner (AGNP or CNP) who values flexibility, relationship-building, and clinical growth. Whether you’re wound care certified or not, we’ll cover the cost of certification so you can enhance your impact in this critical area of care.
Key Responsibilities:
- Deliver hands-on primary care and wound management in senior living settings
- Participate in chronic care management, palliative care planning, and clinical coordination
- Collaborate with interdisciplinary teams, family members, and facility staff
- Ensure timely documentation and professional communication
Qualifications:
- Licensed Nurse Practitioner in Minnesota (AGNP preferred)
- Background in geriatric care, internal medicine, or primary care
- Must have a valid driver’s license and reliable vehicle
- Interest in or experience with wound care and older adult health

- Marines: NP – Nurse Practitioner
