About the Role:

The LVN Home Health Care Coordinator plays a crucial role in ensuring that patients receive high-quality care in the comfort of their homes. Responsible for day to day direct and indirect process of home care intake coordination for Hospice and Home Health. The coordination of services for assigned multidisciplinary clinicians. Responsible for the day to day flow of intake clinical information and coordination of care for patients. Responsible for disseminating client information to clinical staff and monitor clinicians, schedules, patient status and care.

Minimum Qualifications:

  • Current Licensed Vocational Nurse (LVN) license in the state of practice.
  • At least one year of nursing experience, preferably in home health or community health settings.
  • Basic Life Support (BLS) certification.

Preferred Qualifications:

  • Experience with electronic health record (EHR) systems.
  • Bilingual skills to communicate effectively with diverse patient populations. (Spanish Preferred)

Responsibilities:

  • Functions as the agency intake, receiving/accepting referrals from internal and external sources. Serves as a consultant and role model for office staff other clinicians.
  • Coordinates/Communicates with insurances companies for required authorization and pre-certifications for service delivery. Coordinates physician orders for new referrals also keeps physicians informed about patient care needs, plan of treatment development, plan of treatment changes, and outcome measures.
  • Keeps staff nurses informed about client changes to enhance their clinical independence as necessary also utilizes the clinical knowledge of the clinicians to effectively manage department resources and supplies.
  • Reviews orders including verbal orders and updates.
  • Participates in the intake orientation process of new clinical staff and participates in the orientation
  • Functions as a Visit Nurse as required based on business volumes and staffing levels.
  • Adhere to all policies and procedures of the organization and all federal and state laws and performs other duties as directed.

Skills:

Organizational skills are necessary for coordinating schedules and managing multiple patient cases simultaneously. Additionally, proficiency in technology, particularly EHR systems, enhances the efficiency of documentation and communication within the healthcare team. Preferred skills, such as bilingual communication, can significantly improve patient interactions and foster a more inclusive care environment.
