Job Title: Case Manager

Shift: Multiple openings – Day, Evening, and Night shifts available
Compensation

Hourly Pay Range: $80.00 – $90.00 per hour (based on experience)

Estimated Weekly Gross Pay: $3,200 – $3,600

Weekly Stipend: $1,561.20
Position Summary
A healthcare facility in Hyannis, MA is seeking an experienced Case Manager for a 13-week contract assignment. The Case Manager will support the daily operations of the Clinical Resource Department, including care coordination, utilization review, discharge planning, and collaboration with interdisciplinary teams.
This role involves working closely with physicians, nurses, and social services teams to ensure efficient patient care management, regulatory compliance, and continuous process improvement.
Responsibilities

Oversee daily case management operations including care coordination, utilization review, and discharge planning

Collaborate with physicians, nurses, and clinical staff to ensure timely and effective patient care transitions

Participate in patient unit rounds to identify discharge planning needs and resolve care coordination issues

Provide orientation and training support for new staff members including case managers and social workers

Assist in the development of continuing education programs for clinical staff

Monitor program compliance and maintain performance dashboards and reports

Analyze operational metrics and identify opportunities for workflow and process improvements

Ensure adherence to regulatory standards and organizational policies

Support interdisciplinary communication and collaboration to improve patient outcomes
Required Qualifications
-
Bachelor’s Degree in Nursing (BSN) (Master’s Degree preferred)

Active Registered Nurse (RN) License – Massachusetts

Minimum 5 years of Case Management experience in an acute care setting

Minimum 2 years of process improvement experience

Knowledge of CMS regulatory guidelines
-
Strong analytical, organizational, and communication skills
Preferred Qualifications

Certification in Case Management or Clinical Documentation Integrity

Previous leadership or supervisory experience

Experience developing and presenting continuing education programs for clinical staff
