Weekly pay: $2,397–$2,443 per week
Guaranteed hours: 40 hours per week
Shift: 5x8 days, 8:00 am–5:00 pm

Responsibilities
- Conduct initial and ongoing patient assessments in the home setting; develop and update the plan of care in collaboration with the patient and family.
- Administer prescribed medications, monitor responses, and manage IV therapies as ordered; document care and outcomes.
- Complete OASIS data collection and other required documentation to support care planning and reimbursement.
- Provide wound care, including assessment, dressing changes, and adherence to wound management protocols; document progress and coordinate with other care team members.
- Educate patients and caregivers on disease management, medications, safety, and post-acute care; arrange and coordinate services as needed.
- Communicate patient status changes to the supervising clinician and adjust the care plan accordingly; ensure timely, accurate charting and collaboration with the care team.
Requirements
- 2+ years of experience as a home health RN
- OASIS data collection experience
- Wound care experience
- Active Registered Nurse license in Pennsylvania
- Ability to work in the home health setting and travel as assigned
