Assess and monitor patient and family psychosocial needs of Ranken Jordan inpatient population. Support and advocate for patients and families. Act as a liaison between patient/family/caregiver and outside agencies as needed. Participate in the development of the plan of care, discharge plan, and assist in the coordination of discharge from Ranken Jordan. Support the improvement of internal systems, processes, and services that support Ranken Jordan patients and families. In the absence of the Outpatient Social Worker, provide supportive services as needed.
Schedule: Monday - Friday with on call rotation. On call is 7 days evenings and weekend and rotates between 6 social workers.
DUTIES & RESPONSIBILITIES

- Know the strategic plan, mission, and values of Ranken Jordan and in accordance, abide by generally recognized standards of professional ethics. Respect and maintain staff and patient confidentiality.
- Prioritize workload especially during high census or during times of unexpected workload variances.
- Adapt to and communicate effectively with patients/families/caregivers, staff and other customers with diverse backgrounds and belief systems. Complete required documentation, per departmental guidelines and hospital policies. Provide oral or written communication of psychosocial information and patient progress to other departments as needed.
- Collect psychosocial information on potential admissions and refer information to the clinical medical director for review. Provide tours and information to patients/families/caregivers prior to admission. Assist with the admission process.
- Participate in the development of the plan of care, identify barriers to discharge, and assist in the development of the discharge plan.
- Assess and monitor patient/family/caregiver psychosocial needs and provide liaison services as needed. Maintain current information on community resources and health and welfare agencies to make appropriate referrals when assisting in resolving patient/family/caregiver psychosocial issues.
- Provide support and supportive counseling for patient/family/caregiver with their stressors and adjustments due to illness, injury or death and assists patients/families/caregivers with working toward therapeutic and medical goals. Participate in interdisciplinary team meetings and patient care conferences, advocate for patients/families/caregivers and support/assist with self-advocacy efforts.
- Function as part of collaborative interdisciplinary team, participate in problem solving, and present suggestions for improvement that enhance the quality of care and services delivered at Ranken Jordan.
- Participate in the collection of data for hospital programs and services including patient, payor, other customer satisfaction, utilization and program effectiveness and provide information to funding agencies upon request.
- Maintain an acceptable level of social services expertise through participation in in-service education and professional continuing education opportunities. Represent the department and agency at community meetings and collaboratives.
- Share departmental on-call duties.
QUALIFICATIONS:
- The Social Worker must have a master's degree in Social Work from an accredited school of social work
- LCSW/LMSW certification or willingness to pursue LCSW/LMSW certification preferred.
- The Social Worker must have a familiarity with hospital regulations and experience in a medical, hospital, or psychiatric setting.
PHYSICAL DEMANDS:
These include stooping, kneeling, crouching, reaching, grasping, pulling, pushing, standing, carrying, and lifting of light loads (up to 35lb.) as necessary.
