Home Health Physical Therapist (DPT)
Location: Denver Metro Area, CO
Schedule: Monday – Friday, 8:00 AM – 5:00 PM
License Required: Doctor of Physical Therapy (DPT) and active Colorado PT license
Mileage Reimbursement: $0.50 per mile
Productivity Requirement: 30 points per week
Territory Covered: Denver, Adams, Jefferson, Douglas, Boulder, Arapahoe, Broomfield, and Larimer Counties
Note: Therapists are not expected to travel to every county in a single day.
Position Summary
We are seeking a compassionate and motivated Home Health Physical Therapist (DPT) to provide in-home physical therapy services to patients throughout the Denver Metro area. In this role, you will evaluate, develop, and implement individualized treatment plans that help patients achieve their highest level of independence and functional mobility.
Responsibilities
- Perform comprehensive physical therapy evaluations and assessments based on physician orders and referrals.
- Review medical records, diagnoses, precautions, and contraindications to develop appropriate treatment plans.
- Establish measurable goals and regularly reassess patient progress and treatment effectiveness.
- Modify treatment plans as needed to optimize patient outcomes.
- Communicate patient needs and progress with physicians, caregivers, family members, and the interdisciplinary care team.
- Recommend and assist in securing adaptive equipment to improve patient function and safety.
- Maintain timely and accurate clinical, treatment, and billing documentation in compliance with company policies and state/federal regulations.
- Adhere to HIPAA, company compliance standards, and professional practice guidelines.
- Supervise Physical Therapist Assistants (PTAs) and Physical Therapy Aides in accordance with state regulations.
- Participate in staff meetings, continuing education, and professional development activities.
Qualifications
- Doctor of Physical Therapy (DPT) degree required.
- Active Colorado Physical Therapist license required.
- Previous home health experience preferred.
- Experience with OASIS documentation and Homecare Homebase (HCHB) preferred.
- Strong assessment, communication, and clinical documentation skills.
- Ability to work independently and manage a flexible territory.

- Valid driver's license and current auto insurance required.
- Ability to travel throughout the assigned territory.
