As a therapist within our organization, you will report directly to the Clinical Manager and play a vital role in assisting patients to regain their strength, mobility, and independence from the comfort of their own homes. By providing tailored care that emphasizes functional improvement, you will empower individuals to overcome challenges and enhance their quality of life.
Key Responsibilities
- Design and implement prescribed physical therapy treatments for patients in their residences, aiming for the highest possible level of physical function.
- Evaluate and document patients' physical capabilities to aid in physician assessments and inform treatment objectives.
- Create and execute a rehabilitation program aligned with the physician's treatment plan and the goals set by the patient care team.
- Provide clear written care instructions to Physical Therapy Assistants (PTAs) and Home Health Aides, supervising their performance and documentation.
- Record patient observations, interventions, and evaluations in a timely and comprehensive manner.
- Communicate patient progress to the physician, Clinical Manager, care team, patients, and their families, while submitting all necessary evaluations, treatment plans, progress notes, and discharge summaries.
Essential Qualifications
- Active and unrestricted Physical Therapy license.
- Current CPR certification.
- Experience working collaboratively with a healthcare team.
Preferred Qualifications
- At least one year of experience as a Physical Therapist.
Compensation
Pay Range: $59.00 - $83.00 per visit/unit, with an annual base salary between $93,000 and $128,000.
Additional Information
- TB Screening: This position involves direct patient interaction and is part of our Tuberculosis (TB) screening program. Candidates selected for this role will need to undergo TB screening.
- Driving Requirements: This role is included in our driver safety program, necessitating a valid state driver's license and personal vehicle liability insurance. Individuals must maintain vehicle insurance that meets or exceeds state minimum requirements, or $25,000 for bodily injury per person, $25,000 for bodily injury per event, and $10,000 for property damage, whichever is greater.
- Scheduled Weekly Hours: 40 hours per week.
