RN Case Manager

REPORTS TO: Clinical Manager/Alternate Clinical Manager

POSITION SUMMARY:

The purpose of the RN case manager to serve as case manager for Medicaid patients and help maintain stabilization, delay/prevent deterioration of health status, manage chronic conditions, and/or improve health status for patients while performing OASIS start of care, transfer, resumption, and discharge.

QUALIFICATIONS:

• RN License, required
• 2+ year nursing experience, required
• 1+ year Home Health nursing experience, required
• Case manager experience, preferred
• Must demonstrate excellent interpersonal skills; both written and verbal, time management, and Microsoft Office skills

ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY

• Completes start of care OASIS for patients to begin home health services as well as regular re-certifications.
• Initiates the plan of care which establishes patient-centered goals as approved by physicians.
• Responsible for the completion of all home health documentation including admissions, re certifications, discharges, and all related OASIS documentation.
• Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.
