Position Title:Ambulatory CarePharmacist
Employment Type: Permanent

Schedule: Variable days/afternoons (primarily 8-hour day shifts)

Overview:
This dual-role pharmacist position supports both central pharmacy operations and ambulatory care services. Key Responsibilities:

• Review and enter medication orders with accuracy and clinical judgment

• Dispense medications in a timely, safe, and efficient manner

• Provide drug information and clinical remendations as needed

• Conduct ongoing drug therapy monitoring and document findings

• Oversee operations in sterile pounding, including high-risk preparations, narcotic infusions, and chemotherapy

• Perform pharmacist-managed consults and clinical interventions per protocol

• If assigned to ambulatory care: remend or manage therapy via collaborative practice agreements

Ideal Candidate Profile:

• Bachelors in Pharmacy or PharmD

• PGY1 residency or minimum of 3 years of clinical experience

• Active Pennsylvania pharmacist license or eligibility to obtain
