We Are Hiring

Registered Nurse Home Health

Our RNs provide skilled nursing visits, patient assessments, care coordination, medication education, wound care, disease management, post-hospital follow-up, and support for patients recovering from illness, surgery, injury, or a change in condition.

RN responsibilities may include:

• Skilled nursing visits in the patients home
• Start of care, recertification, resumption of care, and discharge assessments
• OASIS documentation and care planning
• Medication reconciliation and patient education
• Wound care and chronic disease management
• Communication with physicians, families, therapists, and the care team
• Helping patients remain safe, independent, and confident at home

Licensed Practical Nurse Home Health

Our LPNs provide hands-on skilled nursing care under the direction of the RN and physician plan of care. This role is ideal for a nurse who enjoys patient education, strong clinical follow-up, and building relationships with patients and families in the home setting.

LPN responsibilities may include:

• Skilled nursing visits in the patients home
• Medication education and treatment follow-up
• Wound care and vital sign monitoring
• Support for chronic disease management
• Documentation of patient progress and changes in condition
• Communication with the RN case manager and clinical leadership team
• Helping patients stay safe and supported at home

Requirements

• Active Virginia RN or LPN license
• Current CPR certification
• Reliable transportation
• Valid drivers license and auto insurance
• Home health experience preferred, but we are willing to train the right candidate
• OASIS experience preferred for RN applicants
