Position Overview
As a Licensed Practical Nurse (LPN), you will play a key role in delivering high-quality, resident-centered care across two Assisted Living and Memory Care communities. You'll collaborate closely with caregivers, leadership, and healthcare providers to ensure safe, compassionate, and personalized care.
Key Responsibilities
- Provide direct nursing care, including medication administration, treatments, and assessments
- Monitor resident health and communicate changes to the care team
- Assist in developing and updating individualized care plans
- Support residents with memory loss through patience, empathy, and engagement
- Build strong relationships with residents, families, and team members
- Maintain accurate and timely clinical documentation
- Mentor and support caregivers and new staff
- Ensure compliance with state regulations and company policies
Qualifications
Required:
- Graduate of an accredited practical nursing program
- Active Minnesota LPN license in good standing
Preferred (not required):
- Assisted Living, Memory Care, or Long-Term Care experience
