The independent facilitator supports the participant in navigating the SDP to achieve a more self-directed life. They are responsible for guiding the person-centered planning process, developing an individualized budget, and advocating for he participant's needs and goals.
Key Responsibilities:
Person-centered planning (PCP): Lead or participate in the PCP process to identify the participant's hopes, dreams, ad long-term goals.
Budget Management: Help develop and manage the Individual Budget and Spending Plan, ensuring funds are allocated to meet the participants goals.
Service Coordinator: Locate and negotiate rates with service providers, community resources, and staff that are not part of the traditional regional center vendor list.
System Navigation: Assist with the administrative complexities of the SDP, including paperwork for the regional center to the planning process.
Advocacy: Attend individual program plan (IPP) and (IEP) meetings to advocate for the participants vision and ensure their voice remains central to the planning.
Qualifications & Requirements:
Training: Preferred
Skills: Strong active listening, negotiation and "out-of-the-box" problem-solving skills.
Knowledge: Familiarity with California's Lanterman Act and Regional Center procedures is highly preferred.
