Setting & Population Served
Our facilities function as Skilled Nursing Facilities (SNFs), offering:
- Long-term care for residents needing continuous skilled assistance and compassionate daily support.
- Short-term, sub-acute rehabilitation for individuals recovering from surgery, illness, or injury.
- Collaborative care through a team-oriented approach involving nursing, therapy, and medical personnel working in unison.
- A diverse patient demographic that includes those with orthopedic, neurological, cardiopulmonary, and post-acute requirements.
The patient mix may vary slightly across different facilities. As a therapy professional, your role will be to assist patients in achieving functional improvements and enhancing their quality of life within a nurturing, patient-focused environment.
Position Summary
The Speech Language Pathologist (SLP) delivers direct rehabilitation services in speech therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.
Education, Experience, and Licensure Requirements
- Graduation from an accredited speech therapy program (minimum MS or MA in SLP).
- ASHA certification (CCCs) OR eligibility for a 9-month Clinical Fellowship Year (CFY) to obtain ASHA certification (CCCs) and/or meet ASHA credentialing standards. Once obtained, ASHA credentialing must be maintained.
- Current registration/licensure in the applicable state, with an active license in good standing throughout employment.
- Preferred one (1) year of experience in post-acute care or a related environment.
- CPR certification required upon hire or to be obtained during orientation, with the necessity to keep it current throughout employment.
Specific Job Requirements

- Proficient in Microsoft Word, Excel, and email communication.
- Coordinate with patients, families, and support departments to effectively address patient needs.
- Exhibit proper body mechanics at all times.
- Ability to make independent decisions when necessary.
- Familiarity with practices, procedures, and regulations governing operations in a post-acute care setting.
- Implement and interpret departmental programs, goals, objectives, policies, and procedures.
- Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation.
- Maintain professional relationships with colleagues, vendors, and others.
- Ensure confidentiality of all proprietary and sensitive information.
- Understand and adhere to organizational policies, including those related to harassment and compliance.
- Exhibit integrity and professionalism by following the organization’s Code of Conduct and completing mandatory training.
Essential Functions
- Establish, evaluate, and adjust realistic, measurable, timely, and functional goals.
- (SLP CCC only) Oversee and assess care provided by SLP CFYs.
- (SLP CFY only) Follow speech therapy treatment plans for patients under the supervision of the SLP CCC, in compliance with Federal and State guidelines.
- Document appropriately and in a timely manner.
- Utilize therapy software accurately and effectively.
- Provide excellent customer service and maintain a positive attitude towards patients.
- Assist in the safe evacuation of patients when necessary.
- Demonstrate reliable and consistent attendance.
- Employ reasoning skills and sound judgment in decision-making.
- Communicate and collaborate effectively within an interdisciplinary team.
- Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during work hours.
- Read, write, speak, and comprehend the English language.
