Our RN Hospice On Call 7 on 7 off is a full-time position. At ExpectCare, you will be responsible for the following:
- Responsible for will responding to telephone requests for assistance and performing home visits for hospice patients and families in their home setting after-hours and on weekends.
- Responsible for answering and responding to incoming calls, referrals, and requests for assistance, completing admissions into care, and making follow-up visits for patients admitted the day prior; as necessary.
- Communicating significant findings, problems, patient’s needs, reports changes in condition or environment to the physician, clinical director and other members of the hospice care team and obtains physician orders as required.
- Responsible for maintaining the flow of communication between in-house staff, field staff and other departments, for coordination of activities and services.
- Participating as a member of the interdisciplinary team in patient care conferences.
•Performs other duties as assigned consistent with skills and training and the mission and goals of ExpectCare Hospice.
Qualifications:
Associate or bachelor’s degree from an accredited nursing school
Strong communication skills (verbally and written)
Hospice experience desired
Prior use of Electronic Medical Records (EMR) preferred
