Are you a compassionate and skilled Physical Therapist looking to make a real impact in patients lives? We are seeking an experienced Physical Therapist to join our dedicated home health team in San Jose, CA. In this role, you'll provide personalized, high-quality care to patients in the comfort of their homes, helping restore mobility, relieve pain, and improve overall function and quality of life.
Key Responsibilities

Conduct physical therapy assessments, diagnostic tests, and ongoing evaluations for patients under a physician-approved Plan of Care.

Develop, review, and update individualized care plans in collaboration with the interdisciplinary healthcare team.

Provide in-home treatments to restore strength, coordination, and body alignment while relieving pain and improving mobility.

Educate and counsel patients and caregivers on therapy needs, symptom management, and safe mobility practices.

Document treatment plans, interventions, and patient outcomes in compliance with agency standards.

Actively participate in care conferences, discharge planning, and quality improvement initiatives.

Supervise Physical Therapy Assistants and Home Health Aides, ensuring the highest level of patient care.

Maintain compliance with state, federal, CHAP, and JCAHO regulations.
Qualifications

Graduate of an accredited Physical Therapy program (APTA-approved).

Current California Physical Therapist license (or eligibility).

Minimum of 2 years of healthcare experience (home health experience preferred).

Current CPR certification.

Strong communication, documentation, and interpersonal skills.

Ability to work independently with excellent time management skills.

Valid drivers license, auto insurance, and reliable transportation.
