Position Overview
As a Home Health Physical Therapist, you will:
- Provide skilled physical therapy care in patients’ homes.
- Follow a per-visit payment plan, with rates that vary based on visit type:
- Start of Care (SOC): highest reimbursement
- Evaluation Visits: base rate
- Routine Visits: standard rates
- Ensure timely and accurate documentation for each visit.
- Receive full support from your assigned manager for scheduling, supplies, and patient-care needs.
- Coverage is based on active partner agencies and patient referrals, not limited to city boundaries.
Compensation
- Competitive per-visit payment plan aligned with local market rates.
- Rates apply to all visits that are completed and documented.
- Training Compensation: $25/hour for up to 12 hours, covering orientation, required training, and shadowing.
Typical Visit Duration (including documentation):
- Evaluation: ~1 hour
- Treatment Visit: ~40–45 minutes
- SOC: ~2.5 hours
- ROC / Discharge: ~1+ hour
