Social Services Director
Come join us at Kahl Home! We are a Faith-Based, Nonprofit, Short-Term Rehabilitation and Skilled Geriatric Care facility sponsored by the Carmelite Sisters for the Aged and Infirm since 1955. Our state-of-the-art services, amenities, and excellent staff make Kahl Home a premier nursing facility in the Quad Cities.
We are seeking a Social Services Director.
- $27 to $31 per hour based on skills and experience.
Social Services Director Qualifications:
- IA Social Work License Required.
- Two years’ experience in a health care setting, rehabilitation or long-term care environment preferred.
- Master’s Degree in Social Work preferred.
Social Services Director Introduction:
Under the supervision of the Administrator, the Social Services Director is responsible for the planning, organizing, implementing, evaluating and directing of the Social Services program in accordance with current, federal, state regulations, industry best practices. Leading Social Work and Case Management staff to assist the residents to attain or maintain the highest practicable physical, mental and psychosocial well-being, navigating the health care system. Responsible for coordination of discharge planning and case management.
Social Services Director Job Summary:
- Guide facility in resident advocacy, protection and promotion of residents’ rights.
- Manage department staff to allocate resources, supervise, plan and implement individual resident plans of care.
- Implement Social Work & Case Management initiatives and training of policies and procedures that govern social services.
- Prepare for the long-term care survey. Interact with State surveyors.
- Demonstrate assessment skills sufficient to evaluate residents’ behavior, to collect data and to evaluate psychosocial needs, risk factors for psychosocial deterioration and residents’ responses to interventions.
- Perform administrative requirements such as completing necessary forms and reports, including MDS and care plans.
- Audit clinical records of residents on a regular basis.
- Spearhead and coordinate interdisciplinary care plan meetings.
- Maintain great relationships with community providers.
- Support Advanced Directives process and work closely with the medical and nursing team to initiate the desired resident/health care proxy’s decisions.
