Hospice Continuity of Care Nurse, Extended Hours (RN)
VNA Health Group Hospice is seeking an experienced Hospice RN – Continuity of Care & Triage to provide clinical support and coordination during evenings, weekends, and holidays. This role serves as the primary resource for hospice field staff, ensuring patients and families receive timely, compassionate, and high-quality care outside of regular business hours.
Responsibilities

Provide clinical guidance and support to hospice field staff.

Triage calls from patients, families, facilities, and staff.

Coordinate visits, staffing assignments, and resources.

Escalate high-risk situations as needed.

Ensure timely and accurate documentation.

Support quality improvement initiatives and regulatory compliance.

Educate, mentor, and support hospice team members.

Collaborate with leadership to ensure continuity of care.
Qualifications
-
Active, unrestricted RN license.

Minimum 2 years of hospice experience required.

On-call triage experience preferred.

Strong clinical judgment and critical thinking skills.

Excellent communication and organizational abilities.
