Care Coordinator (Bilingual Spanish) – CCM, PCM, RPM & RTM Programs

Primary Location: Kissimmee, FL (Hybrid)
Job Type: Full-Time
Pay: Starting at $16/hour · Negotiable up to $24/hour based on experience and overall fit
Employment Structure: Month-to-month contract with opportunity to transition after 90 days into a full-time KaiCare AI employee (benefits eligible)

Role Overview

KaiCare AI is seeking a Bilingual Spanish-Speaking Nationally Certified Clinical Medical Assistant (CCMA) to support our partnership with Care Dermatology.

This role serves as the bridge between patients, providers, and care management services. The Care Coordinator will be responsible for educating patients about available care programs, obtaining consents, enrolling eligible patients, and maintaining an active caseload across Chronic Care Management (CCM), Principal Care Management (PCM), Remote Patient Monitoring (RPM), and Remote Therapeutic Monitoring (RTM) programs.

Key Responsibilities

Patient Enrollment & Consenting

  • Identify and engage eligible patients for CCM, PCM, RPM, and RTM programs
  • Explain program benefits, services, and participation requirements in both English and Spanish
  • Obtain and document patient consent accurately and compliantly
  • Answer patient questions regarding enrollment, services, and program expectations
  • Ensure all enrollment documentation is completed correctly and submitted in a timely manner

Care Coordination & Caseload Management

  • Maintain an active panel of enrolled patients
  • Conduct routine outreach and monthly patient check-ins
  • Support patient adherence to treatment plans and provider recommendations
  • Monitor patient engagement and identify barriers to care
  • Escalate clinical concerns, symptoms, or patient needs to the appropriate clinical team
  • Coordinate with providers and office staff to ensure continuity of care

Program Management

  • Assist with CCM, PCM, RPM, and RTM workflow execution
  • Monitor patient participation and engagement metrics
  • Track patient activity and documentation requirements for billing compliance
  • Ensure monthly care management requirements are met
  • Maintain accurate records within care management platforms and EHR systems

Documentation & Compliance

  • Document all patient interactions accurately and timely
  • Maintain HIPAA compliance and patient confidentiality
  • Complete required notes, task tracking, and care management documentation
  • Ensure all program activities meet regulatory and payer requirements

Required Qualifications

  • Nationally Certified Clinical Medical Assistant (CCMA) certification required
  • Fluent in both English and Spanish (required)
  • Current HIPAA certification or willingness to obtain upon hire
  • Ability to pass a background check and drug screening
  • Minimum 1 year of experience in a clinical, patient-facing healthcare setting
  • Strong communication and relationship-building skills
  • Comfortable speaking with patients by phone and in person
  • Proficient with EHR systems, care management platforms, and Microsoft Office/Google Workspace
  • Ability to independently manage a patient caseload
  • Professional, dependable, and patient-focused

Preferred Qualifications

  • Experience with CCM, PCM, RPM, and RTM programs
  • Dermatology experience
  • Experience with care coordination or chronic disease management
  • Experience using electronic health record systems
  • Experience with EZDerm or similar dermatology EHR platforms
