The Patient Associate Licensed Practical Nurse (LPN) plays a vital role in the daily operations and logistical support for our Hospital-at-Home (HaH) clinical team. As an essential member of the HaH operations group, the LPN acts as the cohesive force that facilitates all hospital logistics leading up to patient admission. This energetic, patient-focused professional ensures a remarkable experience for patients and their families throughout the HaH admission process.
The Patient Associate (LPN) employs nursing practices to deliver patient care under the guidance of the HaH Admitting MD/APP, who oversees the screening, consenting, and admission of all HaH patients. Responsibilities are carried out in alignment with our organization's mission, values, and service standards. The LPN reports directly to the Director of Clinical Operations (DCO) for daily operational matters.
Position Overview:
Full-Time - Licensed Practical Nurse
Hospital-at-Home Program
Key Responsibilities:
- Provide logistical and operational support for the HaH program, acting as the primary hospital-based resource for daily activities leading up to patient admission.
- Ensure patient rights and confidentiality are upheld within the HaH program.
- Conduct HaH assessments as directed by the HaH Admitting MD/APP.
- Facilitate patient screening and enrollment in the HaH program.
- Coordinate all non-clinical tasks as per the “HaH Admission Checklist,” including managing supplies, technology, and equipment necessary for patient admission.
- Maintain accountability for the care of assigned HaH patients and collaborate with other departments.
- Act as a liaison between the HaH Admitting MD/APP and the HaH Hub (virtual command center) as well as hospital services.
- Organize transportation services for all admitted patients in coordination with the HaH Hub.
- Prepare the HaH ‘Take-Home Kit’ and any additional medical supplies or equipment as directed.
- Work collaboratively with hospital staff to ensure a positive experience for patients and families during the admission process.
- Communicate promptly with all HaH team members and respond to requests as they arise.
- Stay informed about the Scope of Practice and Delegation limitations, adhering to organizational policies and procedures.
Administrative & Planning Duties:
- Manage operational support tasks, including inventory management, supply ordering, logistics coordination, and maintenance of HaH equipment.
- Participate in staff meetings, daily team huddles, and other meetings as required.
- Engage in process improvement initiatives within the HaH program and strive for continuous enhancement.
- Support the implementation of service-related and operational initiatives.
- Perform miscellaneous administrative tasks such as copying, email communication, postal support, faxing, and documentation.
Qualifications:
- Education:
- High school diploma or GED equivalent.
- Graduate from an accredited nursing program.
- Licensure:
- Must maintain a current LPN license in the state of employment and fulfill all State Board License requirements.
- Other Skills:
- Strong communication skills and attention to detail.
- Ability to prioritize and organize tasks in a fast-paced environment.
- Proficient in Microsoft Office and Teams, with a general aptitude for learning new systems.
- Commitment to HIPAA confidentiality and privacy policies.
- Preferred Experience:
- At least one year of experience in patient access and delivery systems, or prior clinical operations experience is highly desirable.
