  • Shift: 12-hour nights (Night Shift LPN)\n

  • Schedule: Every other weekend required\n
  • Estimated Weekly Salary: $1,680–$1,770 per week\n

\n\nJob Requirements (LPN / Travel Nurse Qualifications)\n

  • Active Licensed Practical Nurse (LPN) license in the U.S.\n
  • BLS certification through AHA (American Heart Association) required\n
  • Minimum 3 years of LPN experience, including Long Term Care (LTC) experience\n
  • Prior travel nursing / travel LPN experience required\n
  • Comfortable and experienced working with dementia and memory care residents\n
  • Willingness to float to other units as needed to support facility operations\n
  • Ability to work assigned night shifts and every other weekend (no block scheduling)\n
  • Reliable personal or rental vehicle required (public transportation not accessible)\n
  • Vehicles should have all-season or snow tires November–April\n
  • All employment gaps over 90 days must be explained\n
  • Complete work history required for submission (include bed count, EMR, facility type, unit)\n
  • 7-year background check (nationwide, county court, FACIS3, sex offender) within 18 months of start date\n
  • FACIS 3 and VCIC checks within 30 days of start\n

\n\nResponsibilities (Long Term Care LPN)\n

  • Provide compassionate, high-quality nursing care to residents in a long term care and rehabilitation setting\n
  • Administer medications and treatments as ordered and document per facility policy\n
  • Monitor, assess, and document resident conditions; promptly report changes to the interdisciplinary healthcare team\n
  • Collaborate with nurses, CNAs, therapists, social work, and physicians to ensure comprehensive resident care\n
  • Support residents with dementia, behavioral changes, and complex care needs using person-centered approaches\n
  • Float to other units as needed to maintain staffing and continuity of care\n
  • Maintain compliance with facility policies, state regulations, and long term care best practices\n
  • Communicate effectively with residents, families, and staff to promote resident-centered care\n
