Responsibilities

Provide evidence-based physical therapy assessments and treatment plans.

Deliver one-on-one therapy sessions and track patient progress.

Ensure compliance with clinical guidelines, safety standards, and privacy regulations.

Set measurable treatment goals aligned to patient outcomes and timelines.

Maintain accurate, timely documentation of evaluations and progress notes.

Collaborate with healthcare team to coordinate care and referrals.

Monitor and report on PT metrics and territorial progress in pipelines.

Demonstrate reliability by meeting schedules, attendance, and productivity targets.

Educate patients and families on home exercise programs and self-management.

Identify opportunities for professional development and participate in training.

Assist in program development to expand services and improve outcomes.
Experience/Qualifications

Licensed physical therapist with valid state license

Proven experience delivering evidence-based PT assessments and plans

One-on-one therapy sessions with measurable progress tracking

Strong knowledge of clinical guidelines, safety standards, and privacy regs

Accurate, timely documentation of evaluations and progress notes

Ability to set SMART treatment goals and timelines

Collaborative mindset for coordinating care and referrals with healthcare team

Track PT metrics and pipeline progress within a defined territory

Educate patients and families on home exercise programs and self-management

Reliability: consistent attendance, adherence to schedule, and productivity targets

Proactive participation in professional development and training

Contribute to program development to expand services and improve outcomes

On-site, full-time role with ability to travel within territory as needed
