Primary responsibilities: To provide oversight and coordination for the Behavioral Health Services being provided throughout PACE SEMI. This position provides administrative and clinical oversight for the Behavioral Health Clinicians and service line.

  • Provide coordination of services between the Behavioral Health Specialists, Psychiatrists, Pharmacists, Site Leadership and PIC
  • Formal clinical oversite and supervision of all Behavioral Health Specialists.
  • Assure collaboration, communication and support services are aligned at the centers for behavioral health services.
  • Partner with Center Directors and center clinical managers for oversight of behavioral health specialists as they provide service to participants and clinical teams at each center.
  • Ensure collaborative agreements are up to date, within legal scope of practice between BHS and PIC. This is to assist with collaboration within the care teams and for diagnostic clarity.
  • Development of partnerships with Master’s Programs with Behavioral Health Tracts for provision of internships with PACE (this provides a gateway to assist in recruiting BHS).
  • Conduct complex case review with Behavioral Health Specialists, develop and implement journal club for ongoing staff development. Identify high utilizers in the BH population for targeted case review.
  • Conduct monthly/bi-monthly meetings with BHS staff.
  • Collaborate with Behavioral Health Educator to design and develop on -boarding for all staff, competency evaluation of Behavioral Health Staff, on-boarding for Behavioral Staff, AIMS testing, mandatory reporting requirements, use of diagnostic screening tools, motivational interviewing, trauma informed care, Mental Health First Aid, differentiation between behaviors of dementia and behavioral health, appropriate referrals, and other programs identified that will improve Behavioral Health Care.
  • Identify and collaborate with others within PACE to establish community contracts for services not currently provided within PACE i.e. SUD, Suboxone, Methadone, Crisis Screening, 24 hour case management for BH patients, housing for BH/SUD patients. Examples would be Community Mental Health, Team Wellness, COPE, SHAR.
  • Review and update protocols for referrals, warm hand off after referrals, intake evaluations. Work with educator to ensure protocols are consistent across sites.
  • Collaborate with Director of Quality and Medical Director in the development of the Quality Plan as related to Behavioral Health Services.
  • Identifies metrics to be tracked related to Quality Plan:
    • i.e. ED visits for BH, SUD visits to ED, BHS staff turnover, number of significant incidents within programs with BH participants, participant attendance, time of referral to being seen by BHS/Psychiatrist after intake or return after hospitalization, completion of q 6 months BH vital signs completed, functioning of PACE participants on BH medications/psychotropics, retention rate of participants with a BH diagnosis, depression screening/treatment follow up, medication adherence, number of appropriate referrals to BHS. Develop a BH Dashboard with easy tracking of metrics. Accurately identify the number of participants with SUD- alcohol/drug/opiate usage, mild/moderate/severe BH diagnosis.
  • Work proactively with the intake team to help design and implement a plan of care for those with severe BH diagnoses. Ensure that there is a thorough initial assessment and follow-up appointments scheduled. Assist in the creation of Behavioral Plans for disruptive participants.
  • Provides and directs administrative support for the Psychiatrist including increase in client facing hours, documentation by BHS.
  • Reviews all orders for Neuropsych testing and works with ordering physicians to determine which are necessary.
  • Reviews/mentors/audits BHS documentation for clinical relevance, completeness, timeliness.
  • Works with site leaders to identify space for BHS to provide individual and group therapy, group education.
  • Oversees scheduling of BH specialists, site rotation for BHS.
  • Monitors case loads and productivity of BH specialists. Assists with determining if sites need additional or less BH coverage.
  • Ensures compliance with regulatory requirements
  • Collaborates with Site Leadership and Psychiatrist in providing evaluations of BH staff.
  • Collaborates on regular basis with Psychiatrist regarding Behavioral Health work, difficulties and challenges.
  • Ability to travel to all PACE SEMI centers

Requirements:

Qualifications (knowledge, skills, abilities):

  • Full Licensed Master’s prepared Social worker or Registered Professional Nurse, with current Michigan license, and Master's of Science in Nursing with a behavioral health nursing focus and nurse practitioner license.
  • Minimum 5 years' experience in behavioral health preferred
  • Minimum 3 years in a leadership role preferred
  • Experience in program development preferred
  • Demonstrated knowledge and skill in evaluating, planning, implementing, and monitoring patient-centered approaches to care.
  • Knowledge of and compliance with applicable local, state, and federal laws and regulations.
  • Cultural competence and the ability to lead care for a diverse population.
  • Ability to learn and use electronic health record system.
  • Proficient in Microsoft Word, Excel, Outlook, PowerPoint.
  • Positive, flexible, and solution-focused attitude with ability to collaborate effectively in a team setting.
