LCSW (Licensed Clinical Social Worker)– Heart Transplant Program (Outpatient)

13-Week Travel Contract | Day Shift | No Weekends

Location: Portland, OR
Duration: 13 Weeks
Shift: Day 5x8 (8:00 AM – 4:30 PM)
Setting: Outpatient Clinic (Heart Transplant & Advanced Heart Failure)
Charting System: Epic

Key Responsibilities

• Conduct biopsychosocial assessments
• Develop and revise patient-centered plans of care
• Provide counseling, education, and psychosocial support to patients and caregivers
• Coordinate care with internal and external providers
• Support advance directives and long-term care planning
• Assist with community resources, insurance programs, and financial assistance (TANF, income-based programs)
• Document patient progress, education, and care delivery in Epic
• Participate in transplant and heart failure care coordination

Requirements

• Active Oregon LCSW License (Required)
• BLS Certification (Preferred)
• 1+ Year of LCSW Experience
• Experience with outpatient clinics preferred
• Transplant or cardiac experience is a plus
• Strong skills in care coordination, documentation, and interdisciplinary collaboration
• First-time travelers accepted
• Local candidates accepted
