Job DescriptionWe are seeking a dedicated and compassionate Licensed Practical Nurses (LPN) to join our team in a Long-Term Acute Care (LTAC) facility in Clear Lake, TEXAS. LPN's will provide high-quality, patient-centered care to individuals with complex medical conditions requiring extended hospitalization. Working under the supervision of an RN or physician, the LPN will assist with direct patient care, medication administration, monitoring of vital signs, and collaboration with interdisciplinary teams to ensure the best possible patient outcomes.

Schedule: AM & PM shifts
Unit: LTAC Hospital
Starting Hourly Pay: $40.00
What You'll Do:
As an LPN in a Long-Term Acute Care (LTAC) facility, you will play a critical role in providing direct patient care to individuals with complex, high-acuity medical needs. LTAC patients often require extended hospitalization due to conditions such as respiratory failure, chronic wounds, post-surgical complications, and severe infections. The LPN works closely with Registered Nurses (RNs), physicians, and other healthcare professionals to ensure patients receive continuous and specialized care.
- Requirements:
- LPN license in State of Assignment or compact license
- 1 year of Medical Surgical or 1 year of LTAC
- Must have BLS, ACLS (preferred) IV Certification or equivalent
- Immunizations; TB, Covid & Flu

Responsibilities
- Deliver direct nursing care, including wound care, respiratory support, and infection control management.
- Administer medications and treatments as prescribed by physicians.
- Monitor and document patient conditions, reporting any changes to the RN or physician.
- Assist with activities of daily living (ADLs) such as bathing, feeding, and mobility support.
- Maintain accurate and up-to-date patient records.
- Collaborate with interdisciplinary teams, including therapists, dietitians, and social workers, to develop and implement individualized care plans.
- Provide emotional support and education to patients and their families.
- Ensure compliance with all healthcare regulations, infection control policies, and facility protocols.
