Physical Therapist – Outpatient Neurorehabilitation

Acker Physical Therapy & Neuro Rehab is seeking a compassionate, motivated Physical Therapist to join our interdisciplinary rehabilitation team. We specialize in providing individualized, evidence-based care to adults with neurological and complex medical conditions.
\n
This position is ideal for a therapist who enjoys building meaningful relationships with patients, collaborating with other rehabilitation professionals, and helping individuals improve their mobility, independence, safety, and overall quality of life.

Primary Responsibilities
\n
- \n
- Evaluate and treat patients with neurological and complex medical conditions
- Develop individualized plans of care based on each patient’s functional needs and personal goals
- Provide evidence-based interventions addressing strength, balance, coordination, gait, endurance, transfers, functional mobility, and fall prevention
- Complete thorough and timely clinical documentation
- Educate patients, caregivers, and families regarding treatment recommendations, home exercise programs, safety, and functional carryover
- Monitor patient progress and modify treatment plans as appropriate
- Collaborate closely with physical therapists, occupational therapists, speech-language pathologists, physicians, and other healthcare professionals
- Maintain a supportive, encouraging, and patient-centered treatment environment
- Participate in team meetings and contribute to the continued growth of our neurorehabilitation program

Qualifications and Requirements
\n
- \n
- Physical Therapy degree from an accredited physical therapy program
- Current South Carolina Physical Therapy license or eligibility to obtain South Carolina licensure
- Current CPR certification or willingness to obtain certification
- Strong clinical reasoning, communication, and interpersonal skills
- Ability to work both independently and collaboratively as part of an interdisciplinary team
- Commitment to ethical, compassionate, and evidence-based patient care
- Experience in neurological rehabilitation is preferred but not required

- Multiple sclerosis
- Spinal cord injury
- Neuromuscular conditions
- Vestibular and balance disorders
- Gait abnormalities and fall risk
- General weakness or functional decline related to neurological or complex medical diagnoses

Position: Part-Time/PRN
\n
Schedule: Approximately 10-20 hours a week
\n
Location: 144 Leisure Lane, Suite 100, Columbia South Carolina
