Our organization is in search of a dedicated and empathetic Licensed Practical Nurse (LPN) to become a vital part of our Healthcare & Medical Services team. In this position, you will deliver direct, hands-on nursing care to residents within a senior living and rehabilitation environment, aligning with our commitment to patient-centered, high-quality care in a family-oriented setting. As an LPN, you will collaborate with Registered Nurses (RNs), therapists, physicians, and other healthcare professionals to ensure safe, effective, and compassionate support for seniors and adults requiring rehabilitation and long-term care.
Your responsibilities will encompass both routine and complex nursing duties within your professional scope, such as:
- Monitoring and documenting vital signs
- Administering medications and treatments as prescribed
- Performing wound care
- Assisting with pain management
- Observing residents for any changes in their condition
You will also play a key role in developing and updating personalized care plans, promptly reporting any clinical changes to the supervising nurse or healthcare provider, and coordinating care to enhance rehabilitation outcomes and overall quality of life.
In this role, you will foster meaningful connections with residents and their families, offering education, emotional support, and clear communication regarding care plans and daily progress. You will assist with activities of daily living as necessary, uphold resident dignity and independence, and contribute to maintaining a safe, clean, and respectful environment that reflects our dedication to compassionate, patient-focused care.
The LPN will adhere to all infection control, safety, and regulatory standards, accurately documenting care and observations in the electronic medical record while complying with HIPAA and facility policies. You will engage in interdisciplinary team meetings, assist with discharge planning and transitions of care, and support quality improvement initiatives aimed at enhancing clinical outcomes and resident satisfaction.
