Job DescriptionSalary: Experience & Credential Based

PRIMARY PURPOSE:

The New Living Health Care, LLC Diagnostician is a contracted position under the direction of the New Living Health Care Services, LLC Clinical Director. The Diagnostician coordinates efforts with individuals assigned to New Living Health Care to assure that initial or annual diagnostic screenings are done in an efficient and sensitive manner, maintaining confidentiality, and demonstrating good clinical judgment. The Diagnostician is a vital member of the interdisciplinary team and will assist in developing a continuum of care that fully and efficiently meets the needs of the person(s) we serve here at New Living Healthcare.


CORE DUTIES AND RESPONSIBILITIES

  • Compete Diagnostic Assessment with person(s) we serve upon initial intake and annually thereafter.
  • Conduct a face-to-face diagnostic interview with the persons we serve within 24 hours after completion of intake.
  • Demonstrate a working knowledge of the DSM and familiarity with the diagnostic criteria sets of all substance use and mental disorders.
  • Demonstrate cultural sensitivity of the lifestyle choices and background of the population we serve.
  • Demonstrate a caring and compassionate approach to care of the person(s) we serve.
  • Development of the individualized treatment plan for person(s) we serve with SMART goals, objectives, and interventions after completion of Diagnostic Assessment
  • Finalize and submit all required Diagnostic Assessment and treatment plans within 24-48 hours.
  • Complete Interpretative Summary based on assessment data and the diagnostic interview. Interpretative Summary should include Department of Behavioral Health required domains.
  • Treatment plan builder forms are completed accurately, and person(s) centered.
  • Meet with the Clinical Director/Clinical Manager after each Diagnostic Assessment for supervision and Diagnostic Criteria to be added to the medical record.
  • Coordinate with front desk staff to establish appropriate scheduling of the person(s) we serve.
  • Ability to establish and maintain effective working relationships with others.
  • Ability to interact with and relate effectively to individuals at all levels of an organization.
  • Ability to listen to others to facilitate an open exchange of ideas and provide for effective communication.
  • Ability to orally communicate information effectively and accurately.
  • Adhere to a code of ethics that is commensurate with their licensing board of adopted by Department of Behavioral health.
  • Orally communicate information effectively and accurately.
  • Ability to effectively utilize Credible and Google Workspace
  • Maintain confidentiality, at all times, within the framework of HIPPA. Will adhere to established guidelines regarding confidentiality as well as the agency's and professional code of ethics.
  • Maintain accurate and up-to-date records of person(s) we serve as outlined in the agency and NLHC Policies and Procedure Manuals.
  • Other job duties and special projects as assigned.


QUALIFICATIONS

  • Must be a Licensed Social Worker, Licensed Professional Counselor, or a Licensed Registered Nurse in the District of Columbia.
  • Capable of conducting analytical evaluations and communicating effectively with their Clinical Director/Clinical Manager or immediate supervisor.

  • Adhere to dress code, appearance must be neat and clean
  • Complete annual education training requirements
  • Maintain patient confidentiality at all times
  • Be available to work as scheduled
  • Wear identification while on duty.
  • Maintain regulatory requirements, including all state, federal and local regulations.
  • Represent the organization in a positive and professional manner at all times.
  • Comply with all organizational policies and standards regarding ethical business practices.
  • Communicate the mission, ethics, and goals of the organization.
  • Participate in performance improvement and continuous quality improvement activities.
  • Attend regular staff meetings and in-services.
