Job Summary
The Rehabilitation Registered Nurse (RN) provides comprehensive nursing care to patients recovering from illness, injury, surgery, or neurological conditions. The RN works collaboratively with physicians, physical therapists, occupational therapists, speech-language pathologists, case managers, and other healthcare professionals to promote recovery, maximize independence, and improve patients' functional abilities while ensuring safe, high-quality care.
Key Responsibilities
- Assess, plan, implement, and evaluate individualized nursing care for rehabilitation patients.
- Monitor patient progress and identify changes in physical, neurological, and medical conditions.
- Administer medications, IV therapies, wound care, and prescribed treatments.
- Assist patients with mobility, activities of daily living (ADLs), and functional recovery while encouraging independence.
- Provide care for patients recovering from stroke, traumatic brain injury, spinal cord injury, orthopedic surgery, cardiac events, and other disabling conditions.
- Collaborate with interdisciplinary rehabilitation teams to develop and implement individualized care plans.
- Educate patients and families regarding disease management, medications, mobility, adaptive equipment, and discharge planning.
- Perform comprehensive assessments, including pain management, skin integrity, fall prevention, and nutritional status.
- Maintain accurate and timely electronic medical record (EMR) documentation.
- Ensure compliance with infection prevention, patient safety, and hospital policies and procedures.
- Participate in discharge planning and coordinate appropriate post-discharge care and community resources.
