A facility located in Northwestern Wyoming is seeking a part-time Social Worker to join their home health. See details below.
Pay:$28.34-$45.05/hour
Social Worker Opportunity:
- Provide counseling, psychosocial support, and community resource coordination for Hospice and Home Health patients and families.
- Collaborate with the interdisciplinary care team to ensure high-quality, patient-centered care.
- Conduct assessments and deliver bereavement and age-appropriate support services.
Social Worker Requirements:
- Master's degree in Social Work (MSW) required; LCSW preferred or ability to obtain licensure (PCSW considered with approved Wyoming supervision agreement).
- Two years of clinical social work experience in a healthcare setting preferred; hospice, bereavement counseling, volunteer program, and supervisory experience a plus.
- Strong assessment, communication, and interpersonal skills with the ability to provide compassionate support to patients and families.
