Facility Overview

A large, nationally recognized acute care hospital serving Southern Nevada and the Southwest region for nearly 60 years is seeking an experienced Director of Case Management.

This role oversees case management operations across the entire hospital.

Position Summary

The Director of Case Management is responsible for the overall leadership, management, and coordination of the hospital’s Case Management, Social Work, and Chaplain departments.

This executive-level leader ensures compliance with organizational policies and regulatory agencies while integrating patient care coordination and discharge planning processes across hospital departments and external agencies to ensure continuity of care and optimal resource utilization.

The Director is accountable for achieving operational and clinical outcomes through effective leadership of interdisciplinary teams and external stakeholders. The ideal candidate is a strategic, data-driven leader with strong physician and post-acute community relationships.

Key Responsibilities

  • Direct and evaluate departmental operations including staffing models, skill mix, FTE management, onboarding, competencies, and information technology utilization

  • Oversee all core Case Management functions and professional practice standards

  • Ensure adherence to care coordination and discharge planning processes, including timeliness, documentation, and quality

  • Allocate resources effectively to meet productivity and quality benchmarks

  • Analyze performance data and implement sustainable performance improvement initiatives

  • Ensure compliance with internal policies, regulatory requirements, and standard operating procedures

  • Collaborate with executive leadership, including CFO and Division Case Management leadership, to meet organizational goals

  • Build strong working relationships with CMO, CNO, ancillary service leaders, Legal, Ethics & Compliance, and external stakeholders

  • Lead change management initiatives and department-wide strategic planning

  • Participate in rotating on-call coverage as required

Leadership Scope

  • Total FTEs: ~110

  • Direct Reports:

    • 4 Case Management Managers

    • 1 Social Worker Supervisor

    • 1 Administrative Assistant

  • Oversight of Case Management, Social Work, and Chaplain Departments

  • Reports to: VP of Case Management (Division Level)

Required Qualifications

  • Bachelor’s Degree (Required)

  • Master’s Degree in Nursing, Health Administration, or Business Administration (Preferred)

  • Active licensure in one of the following:

    • Registered Nurse (RN)

    • Licensed Clinical Social Worker (LCSW)

    • Licensed Master Social Worker (LMSW)

  • 3+ years of acute care hospital case management experience (Required)

  • 2+ years of case management leadership experience (Required)

  • Demonstrated ability to analyze data, develop action plans, and drive measurable outcomes

  • Strong physician relations and post-acute network collaboration experience

  • Proven leadership, team-building, and change management skills

Compensation & Benefits

  • Salary Range: $135,004 – $202,555 (Based on Experience)

  • Incentive Bonus: 17.5%

  • Sign-On Bonus: $10,000

  • Relocation Assistance: Case-by-case basis

  • Placement Fee: 20% of Offered Annualized Salary
