Job DescriptionMain Responsibilities
- Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions
- Maintains patient database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication
- Ensures that appropriate laboratory tests for monitoring certain high-risk conditions are ordered, evaluated and discussed with appropriate members of the healthcare team
- Obtains, as necessary, and maintains all required documentation in accordance with federal and state regulatory agencies and policy
- Informs the Pharmacist in Charge of medication related errors
- Performs other duties as assigned
Minimum Qualifications
- Bachelor's Degree in Pharmacy is required
- Knowledge of principals related to pharmaceutical preparation and practices
- Candidates need to be fully licensed in PA, NJ and DE or willing to get licensed in all three states
Preferred Qualifications
- Two years' experience as a pharmacist, long-term care pharmacy preferred
- Strong leadership skills
- Detail oriented; goal oriented, team player, oral communication
- A combination of comprehensive therapeutic knowledge, problem-solving skills, and judgment is necessary
- Ability to work cooperatively with others; comprehension of the current standards of pharmacy practice; and understanding of the challenges of professional pharmacy practice
- Ability to master computer/software applications necessary for completing job tasks as required
- Ability to manage multiple employees and workloads at once and meet deadlines
- Excellent verbal and communication skills.
