We are a well-established, family-owned skilled home health agency serving San Antonio for over 30 years. Our organization is known for stability, strong leadership, and a compliance-first clinical culture.

We are currently seeking a Home Health RN or experienced LVN to join our team in an office-based clinical case management role with occasional field support when needed.

This is an excellent opportunity for a nurse who enjoys organization, communication, and clinical coordination, while still maintaining involvement in patient care.

This is not a remote position. The role is primarily office-based with occasional visits in the field when clinical support is needed.

Position Overview

The Clinical Case Manager supports the day-to-day operations of a skilled home health agency by coordinating care, communicating with physicians, and ensuring documentation remains compliant with Medicare and state requirements.

This position plays an important role in maintaining quality care for our patients and supporting the field clinicians.

Responsibilities

Process physician orders, verbal orders, and plans of care
Track and follow up on unsigned physician orders
Communicate with physician offices regarding updates and clarifications
Coordinate with field nurses regarding patient status changes
Monitor hospitalizations and track discharge follow-ups
Ensure timely documentation in the EMR
Participate in interdisciplinary case conferences
Assist in maintaining compliance with Medicare home health regulations

Occasional Field Responsibilities

Perform supervisory visits when required
Conduct urgent or problem-focused visits when needed
Provide clinical support to field staff when escalation is necessary

Qualifications

Required:

Active Texas RN or LVN license in good standing
Minimum 1 year home health experience (Medicare experience preferred)
Experience working with physician orders and plans of care
Comfortable communicating with physicians and clinical staff
Strong organization and documentation skills
Experience with EMR systems
Willingness to occasionally perform field visits

Preferred:

Home health case management experience
OASIS familiarity (RN applicants)
Bilingual (English / Spanish)
