Allied Health Group is hiring Health Coaches and healthcare professionals to conduct in-home preventative care visits and patient health assessments across counties throughout New York State.
This role is ideal for healthcare professionals who enjoy working independently, traveling within their community, and supporting preventative healthcare for patients in their homes.
Assessments are available in every county of New York State, and technicians will work within their selected counties.

• Nursing Students / Pre-Med Students

• Other healthcare-trained candidates
Scope of Work
Health Coaches conduct in-home, in-person healthcare visits on behalf of a medical provider.
Each visit typically lasts approximately 30 minutes.
During visits, Health Coaches may conduct:
• Care management surveys
• Vision screenings
• Hearing screenings
• Dental screenings
Screenings may include one or multiple preventative assessments during the same visit.
Responsibilities
Health Coaches are responsible for:
• Conducting professional in-home patient visits
• Completing patient surveys and preventative health assessments
• Performing basic preventative screenings (vision, hearing, dental)
• Uploading required documentation and photos through company software
• Following HIPAA compliance and infection control protocols
• Managing appointments using company scheduling software
• Coordinating and confirming patient visits
• Traveling within assigned counties to patient homes
Technology Requirements
Candidates must be comfortable using basic technology and mobile software, including:
•Appointment management software
• Uploading visit documentation and photos
• Managing scheduling and patient communication
Training will be provided for the company systems.
Transportation Requirement
Candidates must have a reliable method of transportation and be able to travel to patient homes within their selected counties.
Compensation
This is a 1099 Independent Contractor position.
Health Coaches are paid per completed patient visit.
Typical compensation for a 30-minute visit ranges from $40 – $70 depending on screenings performed.
Pay Structure
• $40 — One screening
• $50 — Two screenings
• $60 — Three screenings
• $70 — Four screenings
