Job Description

$75,400 to $91,300.00

MDS Coordinator – LPN with 3+ Years’ Experience | Training Provided

We’re seeking a detail-oriented and motivated nurse to step into the role of MDS Coordinator. If you’re an LPN with at least 3 years of nursing experience, this is a great opportunity to grow your career in care coordination and compliance.

What You’ll Do

  • Coordinate the timely completion and submission of MDS assessments and care plans

  • Monitor PPS and OBRA assessments, Triggers, and RAPs for accuracy and compliance

  • Facilitate care plan conferences with the interdisciplinary team

  • Maintain and update MDS-related policies and documentation

  • Ensure regulatory compliance, including maintaining 15-month MDS records

  • Assist with resident assessments upon admission

  • Provide nursing support and perform clinical duties as needed

  • Prepare reports, evaluations, and documentation for internal and external use

  • Participate in surveys and inspections by regulatory agencies

  • Contribute to quality assurance initiatives and mentor new staff

  • Participate in the on-call rotation every 3 weeks for a 7-day period

Qualifications

What We’re Looking For

  • Active, unencumbered RN license in the state of Colorado

  • Minimum 2 years of experience in MDS coordination, care planning, or nursing leadership

  • Solid understanding of MDS 3.0, PPS, OBRA, RAPs, and Medicare/Medicaid regulations

  • Strong communication, documentation, and leadership skills

  • Ability to work both independently and collaboratively

  • Proficiency with Microsoft Word, Outlook, Excel, and EMR systems

  • Willingness and ability to be on-call for 7 consecutive days every 3 weeks

  • Commitment to person-centered care and regulatory excellence

Working Conditions & Physical Requirements

  • Light physical effort (lifting up to 20 lbs occasionally)

  • Frequent sitting, standing, bending, and walking

  • Regular interaction with residents, families, and team members

  • Fast-paced environment with occasional interruptions
