The Physical Therapist will provide high-quality care to each assigned patient, resident, or client with daily nursing care and services in accordance with the resident/patient/ or client's assessment and plan of care.

Worksite: Hillside Manor Rehabilitation and Extended Care182-15 Hillside Avenue, Jamaica Estates, NY 11432

Responsibilities:

  1. Conduct comprehensive assessments of residents' physical abilities, mobility, and functional limitations.
  2. Develop individualized treatment plans based on assessment findings, resident goals, and physician referrals.
  3. Implement evidence-based physical therapy interventions to enhance residents' strength, balance, coordination, flexibility, and overall mobility.
  4. Provide therapeutic exercises, manual therapy, gait training, and other modalities to improve residents' physical function and independence.
  5. Collaborate closely with interdisciplinary healthcare professionals, including occupational therapists, nurses, and physicians, to ensure a coordinated approach to resident care.
  6. Evaluate the need for assistive devices, orthotics, and mobility aids to enhance residents' safety and mobility.
  7. Educate residents and their families on proper body mechanics, therapeutic techniques, and home exercise programs to optimize treatment outcomes.
  8. Monitor and document residents' progress throughout the course of therapy, adjusting treatment plans as needed.
  9. Adhere to legal, ethical, and professional standards of physical therapy practice.
  10. Participate in team meetings, case conferences, and in-service training to enhance clinical skills and knowledge.
  11. Stay updated with the latest advancements in physical therapy techniques and contribute to the continuous improvement of clinical practices.

Qualifications:

  1. Valid licensure as a Physical Therapist (PT) in the state of New York.
  2. Strong knowledge of physical therapy principles, assessment techniques, and evidence-based treatment interventions.
