The Primary Care Physician (PCP) within our organization is expected to:
- Be accountable for outcomes: The PCP takes responsibility for delivering strong clinical care and cost-effective outcomes for a panel of up to 450 patients. They recognize the significant impact of building trusting relationships and encouraging behavior changes on patient outcomes.
- Act as a health coach: Rather than merely consulting on illness, the PCP helps patients establish both short- and long-term health goals, collaborates with them to achieve these goals, and regularly follows up to ensure progress.
- Simplify actions: The PCP prioritizes tasks effectively to make behavior changes more achievable for patients and guides their care teams toward performance objectives.
As a value-driven, outcomes-focused organization, we regularly assess the following metrics for each PCP's patient panel to support their success and advancement:
- Patient admissions per thousand
- Utilization of 18-21 appointment slots daily (with new patients counting as 2 slots and follow-ups as 1)
- CGCAHPS (patient experience)
- Closure of clinical gaps
- Medical cost measures
Each PCP will have specific goals related to these metrics and is expected to collaborate with center and market leadership, as well as care teams, to achieve them. Fostering a positive culture of love, accountability, and passion is crucial, as PCPs are leaders within our centers.
The PCP must demonstrate the ability to work both independently and collaboratively with other healthcare professionals. They will closely coordinate with managers and medical directors to ensure adherence to guidelines and participate in risk and quality management initiatives, clinical meetings, and other relevant gatherings that promote patient health and organizational objectives. Compliance with departmental goals, performance standards, regulatory requirements, and quality patient care policies is essential.
KEY JOB DUTIES/RESPONSIBILITIES:
- The PCP independently manages care for patients with both acute and chronic conditions, particularly in older adults.
- They take full responsibility for patient care and outcomes, seeking specialist consultations when necessary while remaining engaged in the patient's comprehensive care.
- Engagement with hospitalists is expected whenever their patients are hospitalized, regardless of the hospitalist's affiliation.
- The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with complex health needs.
- Leading their care team, which includes a medical assistant, care facilitator, and care coordinator, is a key responsibility for the PCP.
- For patients unable to visit the office—whether in hospitals, skilled nursing facilities, long-term care, or homebound—the PCP will collaborate with transitional care teams and other available resources.
- The PCP will actively participate in the management of their center and may cover for other providers as needed. They are also expected to assist in recruiting patients and additional providers for the organization.
- Other duties may be assigned at the discretion of management.

- Proficiency in Microsoft Office Suite, including Word, Excel, PowerPoint, and Outlook, as well as other relevant software.
- Willingness to travel locally as needed, and occasionally regionally or nationally for training or recruitment purposes.
- Fluency in English, both verbal and written; some positions may require proficiency in additional languages, which will be specified during the application process.
- This role necessitates the exercise of independent judgment.
SALARY RANGE:
$221,141 - $315,915
