Job Responsibilities:
- Provide skilled physical therapy services to home health patients in accordance with the physician's plan of care
- Evaluate patients and develop individualized treatment plans
- Educate patients and caregivers on treatment strategies, safety, and home exercise programs
- Document visits and progress accurately and timely in Kinnser EMR
- Collaborate with an interdisciplinary team to ensure coordinated, quality care
Qualifications:
- Current Florida Physical Therapist (PT) license
- Minimum of 1 year of clinical experience (home health preferred but not required)
- Reliable transportation and valid drivers license
- Strong clinical, organizational, and communication skills
- Passion for home-based care and improving patient outcomes
