Pay: $57,000 - $62,000 Annually
Shift: Full Time, 2nd Shift

ResponsibilitiesThe following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.
- Conduct brief biopsychosocial assessments to ensure appropriateness of fit in programming and proper level of care.
- Quickly build and establish rapport with referred clients as well as community stakeholders to ensure best customer experience.
- Use knowledge of the 12 core functions as well as systems theory to promote a brief assessment of person-centered care needs.
- Assist prospective clients in locating and gaining access to necessary services.
- Navigate community relationships to support individuals in receiving information and connection for financial, housing, and other services.
- Determine the client's program eligibility based on client contact, chart review and, other collateral records.
- Work with prospective clients to plan and arrange for service initiation.
- Maintain ongoing communication with prospective clients related to program referrals, admissions, and financial responsibility.
- Verify funding for client admissions.
- Determine client financial responsibility and monitor any changes.
- Work with key stakeholders and prospective clients to ensure seamless transition to programming, including by facilitating transportation and addressing barriers to treatment (social, financial, etc).
- Monitor and manage system waitlist(s).
- Understand system state so as to make decisions and approvals regarding client admissions, payment plans, and information needed for successful integration of care.
- Complete documentation to ensure clarity of client needs.
- Support warm hand-offs to the appropriate care team.
- Build and maintain relationships with referral partners towards enhanced customer service and community representation.
- Perform necessary data entry and maintain accurate documentation.
- Maintain the daily census log.
Required SkillsRequired:
- Bachelor’s in Human Service or Business related field .
- Possess a valid license in the social work or counseling domain ( LSW, LGSW, LISW, LICSW LADC) and/or at least 4 years’ experience in case management or care coordination.
- Prior experience or knowledge of substance use and co-occurring disorders with at least 1 year of work experience in a co-occurring (chemical dependency and mental health) program
- Three or more years’ experience in a chemical dependency facility and 2-3 years of related experience preferred.
- Two or more years of experience in sales, marketing, or business development preferred
- CPR certification, or the ability to obtain certification prior to hire required
- Must know and understand all rules, regulations, and requirements of the assigned program.
- Computer literate with proficiency in documentation and communication tools.
- Flexibility to manage varied and changing responsibilities in a fast-paced environment.
- Ability to work independently with minimal supervision.
- Possess a valid MN driver's license.
